Quick answer

If you searched low testosterone symptoms after a tired month, a quieter libido, or a gym stall, the first job is to separate a feeling from a diagnosis. The Endocrine Society of Australia (Yeap et al., MJA 2016) treats androgen deficiency as a clinical diagnosis with a pathological basis, confirmed by hormone assays. Symptoms alone are not enough. A low number alone is not enough. "Andropause" and "low T" are the words ESA lists under misuse, not under use.

What blood can show is the morning pair: total testosterone with luteinising hormone (LH) and follicle-stimulating hormone (FSH), drawn between 8 am and 10 am. ESA wants that window because testosterone is often lower later in the day. Fasting gives a higher reading. SHBG (sex hormone-binding globulin) can explain a total that looks low or high. Calculated free testosterone is reported on many Australian forms. ESA says it is not recommended for clinical decision making.

An isolated low result often does not hold. ESA cites series in which up to 30% of men with one low testosterone and a normal LH had a normal testosterone on the next morning. Unless one set of results is clearly diagnostic of pathological hypogonadism, at least two morning measurements of testosterone, LH, and FSH are recommended. The useful next step is a comparable morning pair, not a treatment protocol from a search result.

Why symptoms are not a diagnosis

The search is usually "low testosterone symptoms." The college name is androgen deficiency due to pathological hypogonadism: a disorder of the hypothalamic-pituitary-testicular axis that leaves the testes unable to make enough testosterone, enough sperm, or both.

That distinction matters because the same words (tired, flat, less interested in sex, slower in the gym) also describe poor sleep, depression, thyroid disease, iron deficiency, and the metabolic suppression that comes with obesity. ESA is explicit: the positive predictive value of non-specific symptoms is low even when testosterone is reduced. The work is to look for a pathological cause, not to treat a feeling.

Pathological versus functional

Pathological hypogonadism is disease of the testes (high LH and FSH) or of the hypothalamus or pituitary (low or inappropriately normal LH and FSH). Functional suppression is the axis turning down because of illness, obesity, opioids, or glucocorticoids, usually with LH and FSH still in range. ESA says testosterone replacement is warranted in the first group. Evidence for treating the second as a hormone deficiency is limited.

Why "low T" is the wrong label

ESA lists "male menopause," "andropause," "low T," and "late-onset hypogonadism" under misuse: use without a valid medical indication. A gradually lower circulating testosterone with age and comorbidity is common. It is not the same disease as Klinefelter syndrome or a pituitary tumour.

Klinefelter is common and still missed

ESA calls Klinefelter syndrome the commonest chromosomal disorder in males, about one in 580, and a cause of adult androgen deficiency. More than half of men with it go undiagnosed. The finding that would prompt the thought is small testes on examination, not a shopping-list blood panel ordered from a search.

If you came here hoping a number would name the tiredness, this is the honest frame: blood can support or weaken the androgen-deficiency story. It cannot turn a non-specific symptom into a diagnosis on its own.

Symptoms

What people feel, and what colleges name

ESA's post-pubertal list splits into non-specific symptoms, organ-specific changes, and sexual symptoms. Healthy Male, funded by the Australian Government, uses the same shape for GPs. The overlap with ordinary mid-life complaints is the reason a blood test is a filter, not a verdict.

Non-specific, and therefore weak on their own

ESA names lethargy, decreased endurance, low mood, irritability, poor concentration, impaired short-term memory, sleepiness, deteriorating work performance, and hot flushes. Those words also sit on the fatigue, thyroid, and perimenopause pages. They do not pick androgen deficiency out of a waiting room.

Organ-specific signs carry more weight

Reduced muscle mass, increased fat mass, gynaecomastia, osteopenia or a fracture, and small testes on examination are closer to the disease ESA is looking for. Testicular volume in reproductively normal men aged 21 to 35 is usually 15 to 35 mL. That measurement is clinical, not a pathology row.

Sexual symptoms, with a limit ESA states

Decreased libido is on the list. Erectile dysfunction is listed as uncommon as a presenting feature of androgen deficiency, and then only at very low serum testosterone. Healthy Male still says every man presenting with erectile dysfunction should be assessed for androgen deficiency, because it is easy to miss. Neither source treats an erection problem as proof of low testosterone.

Women produce testosterone too, and overlapping energy or libido complaints are real. Elevated female testosterone is the PCOS story. A low female result is interpreted against a much lower reference interval and a different clinical question. This page is the male symptom query. It is not a female testosterone-treatment page.

The tests

What the blood tests actually measure

ESA's first hormonal assessment, once the clinical picture suggests a pathological cause, is circulating testosterone with LH and FSH. SHBG can be informative. Calculated free testosterone is not the decision tool.

Total testosterone

All testosterone in serum: the fraction bound to SHBG, the fraction bound loosely to albumin, and the unbound fraction. Australian labs report it in nmol/L. Immunoassays are what most laboratories run. They have method-dependent bias. ESA prefers mass spectrometry where it is available.

LH and FSH

Luteinising hormone tells the Leydig cells to make testosterone. Follicle-stimulating hormone supports spermatogenesis. High LH and FSH with a low testosterone point to the testes (primary, hypergonadotropic). Low or inappropriately normal LH and FSH with a low testosterone point to the hypothalamus or pituitary (secondary, hypogonadotropic). A normal pair with a clearly normal testosterone makes androgen deficiency unlikely.

SHBG

The liver makes this binding protein. Insulin suppresses it, which is why obesity and insulin resistance lower SHBG and can drag total testosterone down with it. Ageing, hyperthyroidism, liver disease, and some anti-epileptic medicines raise it, which can lift total testosterone without androgen excess. Read the total against the SHBG, not as a lone number.

Calculated free testosterone

Most Australian labs estimate the unbound fraction from total testosterone, SHBG, and albumin rather than measuring it directly. ESA 2016 is plain: there is no evidence that free or bioavailable testosterone is a better measure of androgen status than total testosterone, reference intervals are even less well defined, and calculated free testosterone is not recommended for clinical decision making. RACGP 2014 was more willing to use it in the 8 to 12 nmol/L band. Those are two Australian documents, ten years apart. This page follows the later endocrine statement.

If LH and FSH suggest secondary hypogonadism, the next bloods ESA names are prolactin, iron studies, and a full blood count, looking for a prolactinoma, haemochromatosis, or thalassaemia. Those are not first-line on a symptom search. They belong with a clinician once the morning pair is abnormal in that pattern.

Reading your results

How to read an Australian testosterone printout

Australian pathology reports print each marker with the lab's own name, unit, and reference interval. Field names vary. Match the row, not a remembered US unit or a PBS funding number.

"Testosterone" or "Total testosterone"

In nmol/L. ESA's mass-spectrometry intervals are 10.4 to 30.1 in reproductively normal men aged 21 to 35, 7.4 to 28.0 in unselected young men, and 6.4 to 25.7 in very healthy men aged 70 to 89. Your report will print a different immunoassay interval. Use that interval. A result inside it is not a clean bill of health, and a result just under it is not a diagnosis.

"SHBG"

In nmol/L. There is no single national printed band. A suppressed SHBG in someone with obesity or a high fasting insulin changes how you read a low-normal total. A raised SHBG in older age or liver disease does the opposite.

"Free testosterone" or "Calc. free testo"

Often in pmol/L on Australian forms, sometimes in nmol/L. Do not compare a pmol/L number to a nmol/L blog cut-off. ESA does not want this row used as the decision. If it is the only "low" flag and total testosterone and LH are clearly normal, that is a reason to talk to the clinician, not a reason to start treatment.

"LH" and "FSH"

In IU/L. The pattern matters more than either number. High with a low testosterone: look at the testes. Low or normal with a low testosterone: look at the pituitary and at reversible suppression (illness, opioids, glucocorticoids, obesity). Clearly normal with a clearly normal testosterone: androgen deficiency is unlikely.

The PBS 6 nmol/L line is not your lab interval

From April 2015, PBS-subsidised testosterone for men aged 40 or over without a defined pituitary or testicular disorder needs two morning samples under 6 nmol/L, or 6 to 15 nmol/L with a high LH, plus specialist involvement. That is a subsidy rule. It is not ESA's diagnostic threshold and it is not the reference interval on a Healius or Sonic report.

US pages in ng/dL

An American "300 ng/dL" is about 10.4 nmol/L. Multiply ng/dL by 0.0347. Do not import a US clinic cut-off onto an Australian immunoassay printout.

Reference intervals are population statistics for that assay, not personal targets. Hemexa's own optimal band for male total testosterone is 8.3 to 29 nmol/L, with 6 and 35 nmol/L as the watch and action lines. That is company policy for members, not an ESA diagnostic cut-off, and it sits alongside SHBG and LH on the printout, not instead of them.

Why morning, and why one result is not enough

Two collection facts do most of the damage when a first result looks "low." One is the clock. The other is repeating the draw.

Why 8 am to 10 am

Testosterone follows a diurnal rhythm driven by overnight luteinising-hormone pulses. The serum level is highest in the early morning and falls across the day. ESA therefore wants the sample between 8 am and 10 am. Shift workers and sleep-deprived men are an exception: measure shortly after waking. An afternoon "low" can be the clock, not the testes.

Why fasting

ESA notes that fasting results in higher serum testosterone. RACGP 2014 puts a number on the meal effect: food intake can reduce total testosterone acutely by as much as 25%. If the same form includes glucose, insulin, or lipids, you were fasting anyway. If you were not, say so before anyone treats the number.

Why a second morning

Day-to-day variation is large. ESA cites evidence that up to 30% of men with an isolated low testosterone and a normal LH have a normal testosterone on repeat. Illness, poor sleep, and recent heavy exercise can suppress a single draw. Unless one result is clearly diagnostic of pathological hypogonadism, ESA wants at least two morning measurements of testosterone, LH, and FSH.

PBS funding uses the same two-morning rule. That is a useful coincidence, not a reason to treat the funding number as the diagnosis.

Lookalikes

Other blood-testable causes of the same symptoms

The same symptom list is why a testosterone request often sits next to a fatigue-style filter. A low-looking androgen result does not retire those other tests.

Thyroid

TSH is the first thyroid test. Hyperthyroidism can raise SHBG and distort total testosterone. Hypothyroidism is a common, blood-testable cause of fatigue and low mood that has nothing to do with the testes. See the thyroid blood test guide.

Iron

Ferritin can be low while haemoglobin still looks normal. That is iron deficiency without anaemia, and it is a recognised cause of fatigue. ESA also names iron studies later in a secondary-hypogonadism workup, looking for haemochromatosis. Those are two different iron questions. See the ferritin guide.

Glucose and insulin

Obesity, metabolic syndrome, and type 2 diabetes are associated with lower circulating testosterone and a normal LH. ESA treats that as functional suppression until a pathological cause is found. A 10% weight loss is cited as raising testosterone by about 2 to 3 nmol/L; bariatric-range loss can raise it by more than 10 nmol/L. A morning testosterone pair that comes back clearly normal is a useful result. It moves the next conversation toward the things blood can still catch, and then toward the things it cannot.

Limits

What blood tests cannot show

A clear hormone report is not a dead end. It is the point at which the search has to leave the pathology form.

Sleep apnoea

Obstructive sleep apnoea is a common cause of tiredness, low mood, and a suppressed morning testosterone. Blood cannot diagnose it. ESA treats untreated severe sleep apnoea as a precaution if testosterone treatment is later considered. The test is a sleep study, not another hormone add-on.

Depression and training load

Low mood, irritability, and poor recovery are on ESA's non-specific list and on every mental-health and overtraining list. A testosterone result does not confirm or exclude either. AJGP 2019 says the immediate priority in older men with a low-normal testosterone and no HPT disease is to manage the comorbidity or mood disorder.

Whether you need testosterone treatment

That is a clinical decision after a pathological diagnosis, not a printout decision. ESA says replacement is warranted in proven hypothalamic, pituitary, or testicular disease, and that evidence is limited for older men with chronic disease and no such pathology. This page cannot make that call, and it does not sell treatment.

Fertility

Hypogonadism in its complete form affects sperm as well as testosterone. A serum testosterone does not count sperm. Exogenous testosterone suppresses the axis and can impair fertility in men who still make sperm. That is one reason ESA says treatment should not start before the work-up is complete.

If the morning pair is normal, the next questions are often sleep, mood, medicines, and training. If it is low, the next questions are LH, FSH, SHBG, a repeat morning, and a clinician who can look for a cause. Neither path is a protocol from this page.
